HOME MODIFICATIONS Home modifications are
physical changes made to ones home to
accommodate for the changing needs of the
elderly or disabled to enable aging in place.
SERVICE COORDINATION Includes planning, linking,
advocating, coordinating and monitoring to
assist participants in gaining access to needed
services, financial assistance, housing,
employment, education, social services and
other services
AGING OBRA WAIVERS Aging Home and community-
based program for individuals 60 years of age or
over.
INDEPENDENCE WAIVER Home and community-based
program for individuals with physical
disabilities.
