Introduction
  • Cosmetic dentistry has become extremely popular
    over the last decade or so as the practice has
    become more affordable as techniques have
    advanced. Indeed the market for cosmetic
    dentistry has grown rapidly as people realize
    that gaining that perfect smile is within their
    budget. After all not everyone is blessed with
    great genetics and born with a perfect set of
    teeth.
  • We only have to look to pictures of film stars
    and other celebrities in their youth to see how
    they have later managed to transform their smile.

    career. What is more, a bad looking smile,
    typically self-consciously hidden, can be a major
    impediment. Like it or not a brilliant smile is

    improve their look is to undergo cosmetic
    dentistry. Decades ago this required major

Teeth Whitening
  • Of all the cosmetic techniques in dentistry teeth
    whitening is the most commonly sought after as it
    is inherently painless and does not typically
    require reconstruction or surgery, yet produces
    in most cases startling transformation of the
    patients smile. It will also in still confidence
    and an increased desire to mix socially. As a
    result the cosmetic dentistry practitioners are
    spending more time, capital and effort in
    improving the techniques that result in a bright
    white clean smile.

    only need a daily brush to keep their teeth

    the world would not change the structure of their

    smile. As frustrating as it can be in decades
    gone past they just had to live with this
    infliction. Indeed often if that was the case
    they typically required radical solutions such as
    crowns to replace these unattractive but healthy
    teeth. This is where the techniques for teeth
    whitening have evolved to prevent the loss of
    healthy and often strong teeth just for the sake
    of a winning smile.

The science behind that brilliant smile
  • Teeth colour is inherently genetic just like our
    skin and the colour of our hair. The colour is
    determined by the thickness of the enamel
    covering the core of the tooth called the
    dentine. The dentine is a yellowish colour and
    the surface enamel is most parts translucent.
    This means that those people that genetically
    have thinner layers of enamel covering the teeth
    will naturally experience the colour of the
    underlying dentine showing through, which
    produces a yellow/grey appearance. This does not
    mean that the teeth are less healthy or strong
    just less attractive.
  • However, for most people the shade and tones are
    down to lifestyle and diet. For instance our
    teeth are likely to darken due to consumption of
    tea, coffee, red wine or smoking. Other common
    causes for discoloration are foods such as
    curries, sweet drinks some citrus fruits and of
    course damaged or infected teeth will discolour
    from the inside out. As a rule of thumb most
    dentists suggest that any food or drink that can
    stain clothing has the potential to stain the
    enamel of the teeth. However you may just be
    getting older as everyday wear and tear will
    gradually takes its toll on the thickness off the
    surface enamel letting the underlying dentine
    show more prominently. Also children under eight
    years old taking some antibiotics can develop
    stains such as brown stripes on the teeth which
    can suddenly appear and no amount of brushing can
    remove them. However, this is not just confined
    to children as even adults taking medication can
    sometimes experience similar discolouration of
    the teeth.

  • Consequently there is not one single technique
    that can remedy the condition. Thus because of
    this wide range of causes for teeth
    discolouration an array of techniques have
    emerged to mitigate each one. Therefore treatment
    is dependent on each individuals circumstances.

Surface staining
  • The usual suspects for the surface staining of
    teeth are those lifestyle conditions such as
    eating spaghetti sauces, curries, sweets, fruits
    and vegetables high on chromogens that are
    chemicals that can stick to and stain the tooth
    enamel. Other chemicals such as Tannin found in
    tea and red wine can help chromogens attach to
    the enamel and if the patient is not diligent in
    dental hygiene this can result in a buildup of
    plaque which discolours the teeth and also
    results in a buildup of tartar which makes the
    teeth appear yellow. However fortunately this is
    the most easily remedied cause and diligent
    dental cleaning will remove the surface
    contaminants leaving the teeth once more shiny
    bright.

Self-Treatment
  • For many causes where the problem is through
    simply extrinsic staining there are a variety of
    self-help treatments available across the
    counter. For example there are abrasive tooth
    pastes that remove stubborn stains from the outer
    layer of the enamel. In addition there are
    whitening gels and strips that can be applied to
    the teeth. These are typically peroxide based
    solutions that stain the enamel white but require
    two to three weeks at around thirty minutes a day
    to be affective.

  • However many scenarios are not so simple to
    remedy such as when we consider Intrinsic stains
    as these are stains within the actual dentine
    itself making them far more difficult to remove.
    In this case it is best to consult a dentist who
    can advise on a suitable course of treatment.

Cosmetic Whitening Techniques
  • As we have seen there are many causes for teeth
    discolouration and each requires a different
    approach to treatment. For example some stains
    are extrinsic, which affect only the surface of
    the enamel. In these cases the stains can be
    readily removed using over the counter solutions.
    However for other causes the techniques need to
    be more specific to the root cause.

Laser Treatment
  • Many people try to use self-medication techniques
    to whiten their teeth with varying success. After
    all it is difficult and uncomfortable to apply
    peroxide gel and strips to the teeth for thirty
    minutes a day over at a minimum a two week
    period.
  • Often the effort and diligence required to apply
    over the counter products results in a less than
    satisfactory condition. This is revealed through
    patchy success where some teeth respond to the
    treatment and others are now more visible due to
    their stubborn resistance to treatment. This is
    often because not all teeth are stained due to
    the same reason, some have external staining
    others has deep rooted causes for their
    discolouration.
  • However a critical difference from over the
    counter products or gel trays is that the dentist
    knows what he is doing and so will identify the
    teeth that will respond and importantly those
    that will not respond to surface treatment as the
    staining is inside the tooth and that will
    prevent a common condition to those that
    self-treat of the chess board smile.

Intrinsic Tooth Whitening
  • Dentist will often use laser treatment as it is
    highly effective and can reduce the time and
    discomfort considerably. But dont be fooled into
    thinking this is some sci-fi technique as it uses
    the same basic principles. How it works is that
    the dentist will initially protect the gums by
    covering them with a protective gel and then
    apply the same peroxide solution used in over the
    counter remedies onto the teeth. The difference
    is that the dentist then uses a laser an argon
    or CO2 laser- to heat up and agitate the
    chemicals to perform much more quickly. The
    results are similar with something like eight
    shades of white being achievable but just in a
    much shorter time.

  • But when the cause of the tooth discolouration is
    due to internal rather than external staining a
    dentist will approach the task differently. In
    this case a dentist will use a different
    procedure as the stain is inside the dentine of
    the tooth rather than on the surface enamel.
    Typically they will place a whitening agent
    inside the tooth and then put a temporary filling
    over it. The tooth will be left this way for
    several days and the procedure is often repeated
    until the tooth reaches the correct shade.

But what causes intrinsic staining?
  • Intrinsic stains occur when the inner dentin
    which is the core of the tooth turns dark.
    Intrinsic stains occur for many reasons for
    example exposure to too much fluoride during
    childhood as well as antibiotics. But it can also
    be caused by physical damage that caused internal
    bleeding in the tooth. Treatment for such a
    condition will often require a bit more that
    surface cleaning. It may require root canal
    treatment before whitening, or in more extreme
    cases a crown or veneer to be applied.

Crowns and Veneers
  • Dental veneers are most often used in dentistry
    to provide a cosmetic white coating to the
    patients teeth in order to disguise discoloured
    or misshapen teeth and restore or improve the
    overall look of their smile. The technique used
    requires that a thin piece of porcelain is
    applied to the surface of the tooth which matches
    the desired colour and tone that the patient
    desires. This is a very effective way of covering
    up intrinsically stained teeth that cannot be
    treated any other way. It is typically cheaper
    than crowns and less intrusive. However for long
    term comfort and durability crowns are a better
    option for those with discoloured but otherwise
    healthy teeth.

Crowns
  • Whitening of teeth is not always going to be a
    simple solution. Not all the visible teeth may be
    healthy or in the same state. For example some
    may only have extrinsic stains while others may
    have deep rooted discolouration. In this scenario
    it may be necessary to veneer or crown some teeth
    to make all the teeth match into a perfect smile.
    Crowns are expensive but much cheaper than
    implants so are still a very viable way to whiten
    and brighten your smile. The problem is that
    crowns are very invasive in so much at it
    requires in addition to expense a lot of
    inconvenience and dental work. However the end
    result is far more gratifying and will last for
    ten or more years. In addition having crown
    fitted allows the dentist to reimaging the
    natural layout of your smile. For example they
    can after taking a mould of your jaw make
    significant changes to the angles and position of
    the crowns to straighten and correct any squint
    or misaligned teeth. Crowns therefore may be the
    most expensive other than implants but one of
    the most effective methods of whitening your
    teeth and getting that celebrity smile.

  • Finally we come to probably the flag ship of
    teeth whitening or rather getting that cosmetic

    afford it this is possibly the way to go. You
    will undoubtedly end up with a set of teeth and a
    smile that lights up a room but for most of us it
    is too expensive and intrusive. Instead we have
    to manage our budgets but cosmetic dentistry is

    and celebrities it is hugely affordable. Nowadays
    we no longer have to go through life with the
    teeth we were born with we do have the ability to
