  of the case-to-date, including medical reports,
  tests, treatments and available opinions. Based
  on a complete medical history, functional inquiry
  and physical examination, an opinion regarding
  the diagnosis, treatment and prognosis is
  created.
• Recommendations are made regarding
  rehabilitation, treatment and other case
  management issues relevant to the specific
  referral. In complex cases, a multi-specialty
  physician may be required for the case. The
  purpose of the exam is to determine the nature
  and extent of an injury. The findings of the
  examination are documented in a formal written
  report that address specific questions associated
  with the diagnosis and treatment.
