Go to www.healthpartners.com and click on
Create username and password

Click on I have HealthPartners Insurance

Enter your member information exactly as it
appears on your ID card. Only include middle
initial if it is on your member identification
card.
Need help registering? Click on the Problems
registering? link.

Enter a user name, password, confirm password,
security question and answer. Click Create.

An activation code letter will be mailed to your
home address. This code is needed to use certain
aspects of the website. However you DO NOT need
this code in order to take the health assessment.
Simply click continue.

Welcome Page. Click Accept to Continue.

Enter email address and confirm. You may also
click the check box to opt in to receive
electronic communication.

Click the Get Moving with Healthy Benefits link
on the Medical Plan Services tab

Click on Get Started

Click Take my health assessment. It takes
15-20 minutes to complete.

Click Earn More Points for a summary of your
Health Assessment. Click the link in Step 2 for
