What is a PHA?
  • Readiness and Prevention
  • A PHA is an annual requirement for all active
    duty members that satisfies the Navys need to
    report Individual Medical Readiness (IMR) to the
    Department of Defense (DoD).

What is a PHA?
  • The PHA (1) captures, reviews, verifies, and
    corrects deficiencies in IMR
  • (2) verifies compliance with various elements
    of Deployment Health
  • (3) identifies behavioral risk factors,
    provides individualized counseling based on
    identified risks and assesses current health
    status
  • (4) provides referrals to clinical preventive
    services (colonoscopy, mammography, coronary
    artery disease, screeningetc.) for early
    detection and prevention of chronic illnesses
  • (5) serves as clearance to participate in the
    semi-annual
  • Physical Readiness Test (PRT)

  • - LIMDU referral service
  • - BCA waiver service
  • - Re-enlistment/Separation/Retirement Physical
  • - Overseas/Sea-duty Screening

YOUR RECORD MUST BE ON FILE 96 HOURS PRIOR TO
YOUR APPOINTMENT AT THE CLINIC!!

  • Same Day PHA Clinic
  • 1. Monday Thursday from 0600 to 1430.
  • 2. Appointment Call 953-9039/9042 or come to
    the PHA Appointment Desk located in the
    Main Mall area across from Pharmacy.
  • 3. Anyone who selects this option must be in the
    Health Promotions/PHA Department, NLT 0600
    (located in the New Bldg, Room 1F22) the day of
    your appointment.
  • 4.Fast for 12 hours no eating, drinking or
    chewing tobacco for 12 hours prior to your
    appointment except water and prescription meds.
    Additionally, no alcohol can be consumed 48 hours
    prior to your lab work being drawn.
  • 5. After you are screened by our staff you will
    be assigned to the morning or afternoon clinic.
    You may bring a snack to eat after your labs have
    been drawn.

  • (Part 1 and Part 2 Appointment Required)
  • 1. Part, call Hampton Roads Appointment Center
    1-866-645-4584 or Health Promotions/PHA
    Department at 953-9039/9042 or come to PHA
    Appointment Desk located in the mall area across
    from Pharmacy.
  • 2. Part 1 Appointment identifies discrepancies.
    Arrive ready for blood work to be drawn. Fast,
    12 hrs prior to that appointment. No eating,
    drinking or chewing tobacco 12 hrs prior to
    appointment except water and Rx medications.
    Additionally, no alcohol can be consumed 48 hours
    prior to your lab work being drawn. You may bring
    a snack to eat after your labs have been drawn.
  • 3. Part 2 appointment, seen by a Provider.
    Appointment made by clinic staff only _at_
    953-9039/9042.

    Medical Record Department 96 hours prior to your
    scheduled appointment, keep your appointments,
    and arrive 20 minutes early.

  • Tips for PHA success.
  • 1. PHA Representative (CFL) 8. Keep appoint.
    PHA and PCM
  • 2. Plan early 9. All personnel must see
    HBA
  • 3. Know what to expect 10. PHA once a year.
  • 4. No check-in-box 11. Complete the
    Pre-Appointment
  • 5. Common Sense Rule
    checklist.
  • 6. Come prepared 12. The
    PHA is considered complete
  • 7. Honest medical history ONLYwhen
    all IMR
  • deficiencies are completed!
  • PCM (Primary Care Manager)
  • HBA (Health Benefit Advisor)

  • PARFQs for 100 of all personnel and SF 600s
    for those who require a waivers BCA Not waived
    typed or written for all waivers
  • On the bottom of the PARFQ Cleared or Waived
    pending completion of lab, mammography,
    Optometry, Audiology, PCM visit, etc.
    Nothing written means nothing pending
  • AHLTA SF 600 PHA Form (electronic PHA
    documentation) look for the Outstanding PHA
    Requirements line.
