    including data relevant to Lane County Work Group
    priorities beginning action steps are completed
  • November 06 June 07 Begin implementation of
    new policies, procedures, programs and strategies
  • June 08 All Lane County post data needs to be
    collected and U of Os leadership in this project
    will be completed.

Sample Aim Statement
  • Improve Lane County hospital drug-testing policy
    and procedures so that in 12 months
  • Lane County Hospitals policies and procedures
    regarding who will be tested, how and for what
    will reflect best practices related to the
    testing of mothers and newborns
  • 90 of Lane County hospitals and doctors, when
    surveyed, will report an accurate understanding
    of current hospital policies and procedures for
    testing mothers and newborns
  • 100 of mothers and newborns in Lane County
    tested for illegal substances, as evidenced by
    hospital records, will have received an adequate
    level of testing
  • Guidance
  • Consider program for educating medical staff on
    testing levels
  • Research best practices for screening and testing
    thresholds

Communication/Hospital Leadership
  • Improve communication and collaboration between
    physicians, government and social service
    agencies, families and caregivers related to the
    ongoing care of SEN such that in 12 months
  • 80 of all decisions regarding identified SEN
    hospital discharge plans are made by an
    established and ongoing hospital-based SEN Team
    made up of doctors, hospital social workers,
    Child Welfare staff and representatives of other
    relevant community agencies (EI etc.)
  • 50 of all SEN are discharged with an
    accompanying Continuity of Care Notebook, that
    includes medical history and information and
    records for doctors, caregivers etc. and that is
    created by hospital staff and passed on to Child
    Welfare staff and/or SEN caregivers.
  • the number of SEN in Lane County offered services
    while maintained in their biological families
    will increase by 10
  • the number of Lane County prenatal providers who
    indicate awareness of resources for substance
    abusing mothers will be 90.
