  hospitals we serve
• Focuses on process improvement using outcome
  measures
• To be ever ready for JCAHO

DDP Plan

• It is the policy of OneLegacy to provide donor
  hospitals with a performance plan at the
  beginning of each calendar year so that an
  effective donation process can be in place
• The purpose of the plan is to collaboratively
  develop and implement an effective donation
  process based upon outcome measures
• The plan will provide a system that will monitor
  and develop the donation and improvement process
  ultimately leading to increased organ donation
  opportunities

DDP Plan continued..

• The plan is developed by the assigned OneLegacy
  HSC/HSL at the beginning of each calendar year
  utilizing empirical evidence (data) collected
  throughout the previous year
• The hospital and the HSC/HSL will co-sign the
  plan
• A copy of the plan is given to the hospital
• To ensure maximum donation outcomes the plan is
  review and amended throughout the year as
  indicated by the donation outcomes or trends
