• Formed Sept 12, 2001 by 75 plus individuals
• Four annual scientific conferences
• Initiated patient safety projects 2004
• By-laws and elected leadership 2005
• 2005/06 state recognition through IDPH, IHI and
  IHA
• Largest regional annual scientific conference in
  the country
• Currently over 700 members
