older than 65 - incidence of morbidity of the very old

disorders, falls, incontinence, cardio-vascular
diseases,

environment is of particular importance
Household technology, living environment,
communication- and transport systems, involvement
of older people in social networks, opportunity
to be active into old age Also The expectations
of older people towards health and social service
provision is changing. Older people are an
important part of the social capital of a
community.

- aims at re-orienting health and social services
towards active ageing - is based on the assets of the community and the
individual - bundles the resources in a multisectoral and
multiprofessional working method

- 6 months after the loss of a spouse or close
partner

Guiding question is What do I really want to
do/strengthen/improve, and what keeps me from
doing that right away - organise and identify individual health
promotion package - receive support through the local working group
and the active ageing team.

- advises and supports the work of the advocates
- meets with the actors and stakeholders on a
regular basis
