  • Program Goals
  • To bring together outstanding oncology physicians
    to provide state of the art treatment with the
    support of trained professionals to help patients
    deal with the emotional and psychosocial issues
    of a cancer diagnosis.
  • Patients and families in Harford and Cecil
    Counties deserve access to premier cancer care
    without the inconvenience or discomfort of
    traveling to oncology centers outside of the
    community.

Navigator Programs Across Maryland
  • Most are limited to patients with breast cancer.
    St. Josephs and Anne Arundel Medical Center are
    the only other hospitals that have a nurse
    navigator for other cancer diagnoses.
  • GBMC, St. Agnes and the University of Maryland
    participate in a navigator program co-funded by
    the American Cancer Society (ACS). This is a
    para-professional whose primary focus is to refer
    patients to ACS services.

What makes the Cancer LifeNet Program Unique?
  • Services are provided by trained professionals
  • Services are provided across all therapies
    including chemotherapy, radiation and surgery
  • Services are offered to all cancer patients
    regardless of the type of cancer, no matter where
    they receive their cancer treatment
  • Services are free to all residents of Harford and
    Cecil counties

Accomplishments for 2007
  • Touching more lives in 2007 as demonstrated by
    services offered
  • Added an RN Nurse Navigator to support the Nurse
    Navigator Coordinator
  • Nurse Navigators are expanding their ability to
    meet with patients in the physicians office at
    the time the patient is told they have cancer
  • CLIMB was held at Rising Sun Middle School in
    Cecil County during the school day.
  • Attendance at the support group _at_ HMH is
    progressing well.

Planning/Goals for 2008
  • Complimentary Care Support Group and Education
  • CLIMB will be offered on a weekly basis
  • Increase number of patients seen through the
    Nurse Navigator Program to 40 to 50 patients each
    month
  • Increase the number of trained volunteer
    navigators to 40
  • Continue Community Outreach initiatives and
    educational presentations throughout the county
    in multiple venues

Planning/Goals for 2008
  • Hire Executive Director, Oncology Program
  • Hire 2nd social worker in January and a 3rd
    social worker in September (depending upon
    service volume)
  • Hire the 3rd RN nurse navigator when volumes
    increase to 40 patients per month and a 4th RN
    nurse navigator in September (depending on
    service volume)
  • Hire program receptionist in March
