• Independent inspection carried out on converter
  installation
• Oxygen found to be flowing in the cockpit
• Oxygen disconnected and cockpit opened manually

• Oxy Flow/Mic Tel Test procedure is not in the
  same publication as the regulator installation
  procedure
• Test does not instruct user to reset the oxygen
  to OFF on completion of test

  oxygen systems to complete Hornet LDBO Handling
  Course
• Some contractor personnel had not completed this
