  • Provide needed supplies and equipment
  • Provide goods and services to help relatives,
    friends and others in the provision of your care
  • Manage the placement of people into Long-Term
    Care Homes
  • Provide information about our services, long-term
    care homes and other related health and social
    services
  • Work with other organizations to plan and
