2The Normal CSF Flow
3Hydrocephalus
- Abnormal dilatation of the cerebral ventricular
system - Hydrocephalus should be differentiated from
disorders producing ventricular enlargement
secondary to cerebral atrophy. - Classified into
- Communicating Hydrocephalus
- Non communicating Hydrocephalus
4Characterisics of Hydrocephalus
- Ventricular enlargement disproportionate to the
degree of sulcal widening - Enlagement of temporal horne
- Periventricular fluid 2ry to transependymal CSF
permeation - Enlarged 3rd ventricle with large suprapineal and
chiasmatic receses - In children lt 2 years the head circumference is
often the best distinguishing feature between
hydrocephalus and atrophy.
5Hydrocephalic Brain
Normal CT Brain
6Subependymal CSF permeation
7Non-Communicating Hydrocephalus (Intraventricular
obstruction)
- Ventricular dilatation caused by intraventricular
obstruction at or above the the outlet foramina
of the 4th ventricle - Causes
- Lateral ventricle
- Foramen of Monro
- 3rd ventricle
- Aqueduct of sylvius
- 4th ventricle
8Lateral Ventriclar causes
- Intrinsic tumor ependymoma
- Ventriculitis due to intraventricular adhesions
- Extraventricular Tumour mass effect from large
parenchymal mass
9Ventriculitis case 1
Ventriculitis case 2
10Neuroepithelial Cyst
11Obstruction of Foramen of Monro
- Tumour colloid cyst , subependymal giant cell
astrocytoma - Ventriculitis
- Haemorrhage fresh clot , or adhesive
arachnoiditis - Cerebral swelling with subfalcine herniation
12Subependymal Giant cell astrocytoma
13Subependymal giant cell astrocytoma
14Central Neuocytoma
15Colloid cyst
16Colloid cyst
17Subfalcine Herniation

19Causes of Thid Ventricle Obstruction
- Intraventricular Tumor
- Extraventricular tumour Pituitary adenoma ,
craniopharyngioma, arachnoid cyst.
20Pituitary Adenoma
21Craniopharyngioma
22Causes of Cerebral Aqueduct of Sylvius
Obstruction
- Developmental aqueduct stenosis
- Intraventricular tumour epndymal seedling
- Extraventricular Tumour Pineal Body tumour
- Ventriculitis
- Haemorrhage
23Aqueduct Stenosis
24Pineal Body Tumours
25Pineal Body Tumour
264th Ventricle obstruction
- Intraventricular Tumour Ependymoma, metastases
- Extraventricular Tumours medulloblastoma,
haemangioblastoma, cerebellopontine angle tumours
(acoutic neuroma) , meningioma - Outflow obstrction infection (TB), SAH,
leptomeningeal malignancy
27Ependymoma
28Medulloblastoma
29Cerebellar Haemangioblastoma
30Intraventricular Haemorrhage
31Acoustic Neuroma
32Communicating Hydrocephalus
- There is free flow throughout the ventricular
system. - Impaired CSF resorption by the arachnoid
granulation accounts for majority of cases - SAH
- Infectious meningitis
- Malignant meningitis
- Granulomatous mningitis TB , sarcoidosis
- Altered venous Dynamics Vein of Galen
malformation, venous obstruction
