• Employee Benefits Summary booklet provided by
  the district

Instructions

• Refer to the Wylie ISD Employee Benefits
  Summary, the TRS-ActiveCare Enrollment Guide,
  and the instructions at the start of the Web
  Benefits Enrollment Process for detailed
  information in completing your enrollment.

Processing or Confirming Enrollment

• The Web Benefits Enrollment screens will be
  populated with your current benefit choices
• If you make changes, waive/decline
  TRS-ActiveCare, or elect Medical Reimbursement or
  Dependent Care Reimbursement, forms are required
  and will pop up. Pop-up forms need to be
  MINIMIZED in order to finish your web selection,
  then maximized to complete, print and sign the
  required forms.

FINAL STEP IN THE ENROLLMENT PROCESS

• Sign and date the forms
• Send the forms along with a copy of the Benefits
  Summary page to the Benefits Department no later
  than
• Wednesday, May 24, 2006
