7Aetna Student Health Insurance Plan
- Aetna preferred provider network
- 20 co-pay
- Referral required except for annual gyn, routine
obstetric care, emergency room visits, and
working/traveling 50 miles away. If the ER
refers you to a specialist for follow-up, you
need a referral from us! - Covers other services
- Lab, x-ray and other studies, pharmacy, emergency
care, prescriptions, hospitalization, specialists
- National and international coverage
- If you waive the Student Insurance, you must
repeat your waiver each insurance year (begins
August 17th). You can waive online on the
Student Health Service Website.
8CUMC Insurance Waiver Criteria
- Minimum of 500,000 lifetime benefits per
accident/illness - Covers you overseas
- Covers at least 80 of hospital fees
- Annual deductible 1000 or less
- Out-of-pocket maximum no more than 5000
- Ability to obtain non-emergent care, emergency
care and hospitalization within 50 miles - Insurance company has a US address, telephone
number and claims processing unit.
9CUMC Insurance Waiver Criteria
- Ability to obtain non-emergent care, emergency
care and hospitalization within 50 miles - Inpatient coverage of mental health and substance
abuse of at least 30 days At least 20 visits for
outpatient coverage annually - Coverage extends for the entire school year,
including summers - Plan covers pre-existing conditions at the time
of CUMC enrollment - Plan covers athletic injuries
- Student signs a financial responsibility

- Select Waive Insurance
- Follow the instructions
- 5. Save your SSOL receipt (print it out) it is
the only proof that you waived should a waiver
not go through!
