43rd Party Fatality 10 yr old boy 12/07/05
- What happened ?A PAJV tanker on the way to
Al-Qabil. In Izz village, Nawaf Abdullah Al
Harthy, a 10 yr old boy coming out of an alley on
a bicycle, crossed the road without stopping. The
driver tried to avoid him. He was not successful.
Nawaf was hit. Result - Nawaf suffered severe head injuries and died
before reaching Ibra Hospital. - Learning
- Defensive driving speed in housing area. Slow
down! Expect the unexpected! See these 2 short

road use and crossing. - Make your child wear a helmet when using bikes
or skate boards.
5LTI 26 27 RTA - Head on! 07/07/05
- What happened ?A PAJV pickup driving along the
pipeline Right of Way approached a small hill-top
and had a head on collision with a oncoming third
party land cruiser. - Result
- Driver broken leg
- Passenger broken arm
- 3 people MTC/FAC
- Learning
- A classic mistake of poor defensive driving
both drivers were unable to see oncoming
cars because of the hill-top. Both should
have slowed down and kept to the far right.
They did not. - Seatbelts save lives!
6LTI 25 Chemical Burns 07/07/05
- What happened ?
- The PDO operator was pumping corrosion inhibitor
from a drum into a tank. The discharge hose of
the pump detached and chemical sprayed over the
helpers body. - Result
- Chemical burns shoulder, back arm
- off work for at least 2 wks
- Learning
- Check all pressure connection before starting
a pump! - Stay as much as possible away from pump
discharge and pressure piping

also! Noise Induced Hearing Loss (NIHL) 6 cases
for 2005
- Whats happening?
- Workers exposed to excessive noise 85 dB(A) for
long periods or - peak noises 140 dB(C) without wearing their
hearing PPE. - Results
- Six PDO staff diagnosed with NIHL since Jan/05.
16 cases in 2004. - How to avoid a recurrence
- Identify noisy tasks (HRA) or areas/equipment
noise mapping - Install quieter equipment (
- Engineer out or isolate/contain noisy equipment
- Regularly maintain equipment to keep noise levels
below 80 dB(A) - Place signs to wear hearing PPE in areas 85
dB(A) - In the interim, correctly fit wear ear plugs or
ear muffs 100 of the time when working in noisy
areas or performing noisy tasksFor more

8LTI 24 Fall from ladder 02/07/05
- What happened ?A Schlumberger welder was
standing on a ladder to drill a hole in a
portacabin. The electrical technician was to help
him and cool off the drill-bit by spraying water.
In order to reach he climbed on another ladder.
When he got to the second step he fell over. - Result
- Broken wrist
- Off work for 4 weeks
- Learning
- Use a proper stable work platform
- Get somebody to help and hold the ladder
- Note spraying water near a 240V electric
drill might not be a good idea!
9LTI 23 Broken Finger 01/07/05
- What happened ?Dalma driller and assistant
attempted to screw a 2" union connection. The
line suddenly slipped down a hole and the hammer
union crushed the drillers finger against the
floor plate. - Result
- Broken finger off work for 3 weeks
- Learning
- Think what if look for pinch-points
- Secure pipe-work before working on it
10LTI 22 Crushed Finger 27/06/05
- What happened ?The NDSC roustabout-foreman was
setting out pipe stands next to the catwalk in
order to lay-down casing joints. The forklift had
raised one end of the stand off the ground so
that a piece of wood could be placed under it for
leveling. The forklift lowered the rack before
the foreman could remove his hand. His index
finger got crushed between the steel rack and the
piece of wood. - Result
- Crushed/broken finger

also! Mouse Disease/RSI (Repetitive Strain
Injuries)
- Whats happening?As staff are increasingly desk
bound, repetitive strain injuries (RSI) and
musculoskeletal disorders (MSD) from static
postures are increasing. Power PC users such as
geologists and seismologists, are heavy mouse
users working for extended periods without breaks
or a change in task. Workstations are often not
set up correctly. - Result
- 3 cases of mouse disease this year. One them
was off for 5 days and needed work change. Others
experience discomfort which may lead to permanent
disabilities if work practices are not modified.
This also affects performance output. - How to avoid a recurrence
- Follow the Health Webpage Guidance Material to
set up your workstation correctly - Get advice from your HSE/Ergonomics Focal Point
or the MSE/3 Team - When not using your mouse - dont continue to
hold it. Let go relax your hands. - Sit in close to your desk and bend your elbow
- Modify your behaviour to break up your
activities mouse use every 45 minutes - Rotate hands if using the mouse for extended
periods using Control Panel - Do the stretching exercises
12LTI 21 Broken arm 27/06/05
- What happened ?An Al Turki pipe fitter was
trying to climb over a 28 pipe. He lost
control, slipped, fell down and broke his arm. - Result
- Broken arm
- 3-4 wks off work
- Learning
- Review/provide worksite access and discuss
with crew in tool box talk - Provide (temporary) steps where required
13LTI 20 Chemical burns on leg 22/06/05
- What happened ?The MBPS Assistant Driller was
taking a sample from the mixing tank containing
concentrated Calcium Chloride. His foot slipped
and his leg immersed up to his knee in the
chemical solution. CaCl is a severe irritant to
skin. - Result
- 1st and 2nd degree burns from the knee down.
- at least 1 wk off work

14LTI 19 Rollover-broken arm 17/06/05
- What happened?
- A Fahud Desert Trading articulated water tanker
with a half full tank of water tipped over at a
T- junction. - Result
- Helper broke his arm Nerve damaged - possible
loss of hand movement - 8 weeks off work
- Learning
- Slow down
- Slow down even further when the tank is half
full sloshing !

- What happened?
- A team was given shrimps during a BBQ as a reward
for their recent good performances. - Result
- 13 staff developed food poisoning and several
experienced an allergic reaction to the shrimp.
They were unable to work and needed medical
treatment. Some of them were off work for several
days. - How to avoid a recurrence
- Source food from a reputable supplier
- Always maintain temperature control 60C
for hot food frozen foods - Follow good food handling practices
- Prevent cross contamination (raw from ready to
eat) - Wash hands before handling food
- Catering contractors to consider potential
allergies of clients. e.g. nuts, seafood,
honey, certain preservatives.
16LTI 18 Four 3rd Party LTIs (RTA 09/06/05)
- What happened?
- A PAJV pick-up with a driver and a technician was
travelling on the Muscat-Nizwa highway when the
driver realised he had missed the junction
towards Sur/Qabil. He stopped a few hundred
meters further on the side of the road and
decided to reverse back to the junction along the
hard shoulder. In doing so the vehicle crossed
the line into the main highway lane. A 3rd party
vehicle (with a family of 5 on board) arriving at
high speed was unable to avoid the pick-up and
crashed into it. All were wearing a seat belt. - Result
- 2 children severely injured (ICU)
- Mother and one child broken arm
- Contractor driver LTI (concussion)
- Learning
- Never ever reverse on a highway!!
- Seatbelts for ALL!!
17LTI 17 Burns to Legs 03/06/05
- What happened?
- A PDO earthmoving staff noted high engine temp on
his bulldozer. He decided to check the water
level in the radiator by opening the radiator
cap. He tried with his hands and then kicked it
with his foot. The cap opened and boiling hot
water and steam gushed out and sprayed his legs.
He was lucky in a sense that he did not fall from
the 2.5 m high bulldozer. - Result
- First degree burns to both legs
- 2 weeks off work

- Never open a hot radiator
- Carefully open the cap when cold
18LTI 16 Head on collision 01/06/05
- What happened?
- A Truckoman light canter truck traveling on the
Muscat to Fahud blacktop road collided head-on
with a pick-up coming from the opposite direction
(blacktop road). - Result
- Canter driver LTI deep cuts over his face
- Pick-up driver RWC

- Defensive driving
- Be vigilant of oncoming traffic
19LTI 15 Fatal Injury 31/05/05
What happened? Ahmed al Balushi, a 22 year old
NDSC derrick-man, was man-riding in the mast. He
died when he fell from a height of 35m onto the
rig floor. Our thoughts are with his family,
colleagues and friends.

- What happened?
- Working for 6 hours, a worker developed muscle
cramps. - Result
- The man was affected by the heat and fluid/salt
loss of as a result of working in hot, humid
conditions for extended periods. He was taken to
the clinic and put on a intravenous drip. - How to avoid a recurrence
- Allow workers two weeks to acclimatise to the
conditions especially after returning from leave - Provide plenty of cool potable water
Supplements may be provided especially to make it
more tasty - Take many breaks in shaded or air conditioned
areas - Wear light, natural fibre clothing e.g. 100
cotton - Plan work for the cooler parts of the day or even
night
21LTI 14 Broken arm 28/05/05
- What happened?
- The Shiv-Vani rig was running casing. With 40
tons of casing hanging from the traveling block
the steel hoist wire-rope failed. The traveling
block came crashing down from a height of about
8m. 3 of the floor man were hit by recoiling wire
rope and flying wire pieces. - Result
- 1x LTI broken arm 4 weeks off work
- 1x MTC and 1x FAC

- What happened?

- What happened?
- A Muscat Gas propane tanker driving from Muscat

- What happened?
- While changing a flat rig tire, the 2.5 ton wheel
suddenly came off and fell over. 2 ODE mechanics
jumped away but were hit by the tire. A third one
passed through the centre of the wheel he was
very lucky! - Result
