26APPLICATION FOR CHANGE IN CATEGORY
- Applicant prepare Letter of Application for a
change in category. - 2. Submits Application Letter to the BLR thru the
respective Regional health Office under whose
jurisdiction the applicants locality falls with
the following supporting documents - a. Floor plans of the proposed expansion (3
copies) including a copy of the existing
structure properly marked.
27- b. BLR Form No. 4-A (Statistical Report) for
the past six (6) months. - c. BLR Form No. 4-B (Updated List of Technical
Equipment and Instruments). - d. BLR Form No. 4-C (List of Personnel).
Includes PRC ID card numbers and signatures of
personnel. List must be notarized. - e. Feasibility Study.
- f. Indorsement from the Regional Health
director.
28- 3. BLR COMMITTEE ON Evaluation and Review of
Application for Hospital Establishment, Bed
Increase and change in Category evaluates all
supporting documents and submit its
recommendations to the BLR Director. - If all the requirements conformed with the
Hospital Standards and requirements prescribed by
the DOH, a permit to construct is issued by the
BLR. - BLR prepare Letter of Indorsement to the RHO
informing the latter, the outcome of the
application.
29- 6. Upon completion of the building construction,
the owner requests BLR Inspection Team to inspect
the finished building. (Please refer to
Checklist 3 for the requirements for an
application for registration and application of
licenses to operate a hospital).
30REQUIREMENTS FOR CHANGE IN CATEGORY
