3Purpose
- To immediately separate lead poisoned children
from exposure to lead-based paint hazards, lead
dust hazards, and lead soil hazards found in
their primary residence until hazards are
eliminated
4Who?
- Poisoned children and members of the childs
household (Poisoned child is defined as a child
with blood lead of 20ug/dL or persistent blood
lead of 15-19ug/dL) - Head of household applies for assistance
5Receiving Dwelling
- Temporary relocation hotel/motel, shelter, or
short-term rental - Permanent relocation lead safe dwelling
6ELPR Program will pay Temporary Relocation
- Temporary Relocation actual costs of the
temporary relocation such as moving services,
temporary storage and transportation or a fixed
payment of 1,000 per household plus a 400
dislocation allowance.
7ELPR Program will pay Permanent Relocation
- Costs associated with the implementation of an
approved relocation/housing plan including
security deposit, rent or rent subsidies
8Send with ELPR Program Application
- Copy of hazard assessment conducted by the local
health department of the childs primary
residence - Copy of notice of violation/order to abate
lead-based paint hazards
9Send with ELPR Program Application
- Copy of medical opinion issued by the Health
Official, primary case physician or attending
physician that states that the child must be
relocated due to ongoing exposure to lead-based
paint hazards
10Following ELPR Program Application Submission
- Evidence the receiving dwelling is lead safe
- For permanent relocation cases an approved
relocation/housing plan

other issues like distance to bus routes,
location of day care, school, job
