Introduction
  • Fulfilling the Promises 2001, states that by
2010 services for people with learning
    disabilities in Wales should
  • provide comprehensive and integrated
services that will effectively support people to
achieve social inclusion in all aspects of life
    and society in Wales
  • Through Flexibilities monies the Flintshire
Learning Disability Service appointed a Project
Manager in October 2004, to take forward the
integration of Health and Adult Social Care. It
is envisaged that by April 2006 Flintshire will
have in operation a new Learning Disability
    Integrated Care Management Service.

3
Towards Integration
  • Integrated Working is about Partnership
  • Building trust
  • Recognising differences
  • Understanding what is important to ALL

4
Goal and Objectives
  • Integrated Learning Disability Team
  • Seamless Service - people have all their needs
and aspirations considered at the same time, to
    help avoid crises and duplication
  • A more managed system - partner agencies are able
to concentrate on what they do best and have a
better understanding of who else can help
  • Leaders model partnership working and look beyond
the needs of their own organisation to find
    solutions
  • A shared vision including actions, resources
    risks
  • Improved outcomes for service users

5
Improve Outcomes for Service Users by
  • using a person centred approach
  • service users taking control of their lives
  • improving communication
  • having a greater coordinated accessible team
  • unique and flexible services
  • timely effective service response
  • doing things differently

  • Have a foundation in values principles

7
How Did We Get Here?
  • All Wales Strategy
  • NHS Community Care Services Act 1990
  • The Health Act 1999
  • Local Government Act 1999
  • Numerous re-organisations
  • Commitment of individuals
  • and now Fulfilling the Promises

8
Why Bother?
  • Government directive Health Act, Fulfilling the
    Promises
  • Integration in name only will result in
    disruption and increased miscommunication between
    agencies and consequently promote damaging
outcomes for service users and families
  • The threat is if we do not grasp the challenge of
shared authority, we risk the representatives of
the various parties retreating or demoting the
    role of the partnership.
  • One way to address this is to have clear and
    unambiguous partnership agreements, setting out
processes and business terms, including how to
address the potential breakdown of relationships.

9
Action Plan
  • Analysis of current situation operational
    strategic
  • Consultation teams, groups and on an individual
    basis
  • Prepare for a long-term programme of change
  • An integrated approach to information, staff
    support and organisational development clarity
    of roles responsibilities
  • Joint strategic commissioning
  • Pooled budgets - will ultimately change
    cultures

10
Conclusion
  • No matter how procedurally sound a Partnership
agreement may be, it can never make up for good
    relationships.
  • Effective partnerships are built on secure and
trusting relationships not the other way round
