analysis with benchmark and other pooled data.

Patient Info - Edit This screen displays the proc
ess used to enter patient information into the
database. Through the use of drop-down menus an
d look-up tables, users can easily navigate
through the application. This methodology of dat
a entry is maintained throughout the application.

Pre-Operation Workflow The screen offers bariatri
c programs the opportunity to identify,
selectively assign and track patient specific
tasks required for completion prior to scheduling
surgery. A subset of this information can then be
shared with the patient in print form or via the
patient portal. Reminder lists can be generated i
dentifying remaining tasks to be completed.

Operation Details The screen is used to record th
e surgical procedure. Options like surgery type
are maintained in a look-up table using the
System Settings screen. This allows the
administrator to add or remove different
operation types. Tables that can be customized i
nclude Operation Type, Approach, Surgeon,
Assistant and others.

Patient Visits This screen displays a list of pat
ient visits. It organizes the visits by date and
automatically calculates Body Mass Index (BMI),
Excess Weigh Loss (EWL) and Percent of Excess
Weight Loss (EWL). It also provides access to
co-morbidities, laboratory results, images,
forms, medications and band adjustments.
Previous medical history include previous
procedures, weight loss programs and family
history.

Patient Events This screen displays a list of pat
ient visits. It organizes the visits by date and
automatically calculates Body Mass Index (BMI),
Excess Weigh Loss (EWL) and Percent of Excess
Weight Loss (EWL). It also provides access to
co-morbidities, laboratory results, images,
forms, medications and band adjustments.

procedures, weight loss programs and family

Visit Schedule This screen displays the suggested
schedule for patient follow-up visits and
identifies if they were performed or not.
Administrators can specify the frequency of
follow-up visits, enabling the application to
identify the date ranges where visits should
occur.

Data Graphs The application can display graphs re
presenting weight, BMI, EWL, EWL and Band
Adjustments when applicable. Graph types include
bar or line formats either in 2D or 3D mode,

number of custom fields can be defined, allowing

What is eMD Patient Portal Module? The
Patient Portal Module allows authorized patients
to access specific areas of the database and pe
rform tasks such as registration and virtual
visits. The portal is intended to establish a
closer relationship with the patient and
allow access to historic weight changes while
providing tools for the patient to track food c
onsumption and exercise activities.
When performing a virtual visit, the patient c
an be asked to indicate present
weight, perform surveys, update status of
co-morbidities and update demographic informati
on.

Features My Clinic Log of visits performed at
the clinic with associated measurements such as
weight, BMI, Excess Weight Loss (EWL) values and
percentages. Also provides ability to do clinic
virtual visits to report weight changes.
My Journal Daily log for weight changes and pe
rsonal notes. My Diet Daily food journal that
records food types and calculates nutrient
values according to the USDA SR-16 database.
My Exercise Daily log for exercise activities
which calculates the consumption of calories.
My Info Summary screen displaying values and c
harts of weight goals, progress, food intake and
exercise. Messaging Provides the ability to
exchange private and secure messages between
clinical staff and patients. Links Provides l
inks defined by the clinic to specific web sites,
such as support groups and specialized food
suppliers.

Pre-Operation Workflow This screen displays a lis
t of tasks needed to be completed before surgery
can be scheduled.

Virtual Visits The Patient Portal allows the pati
ent to perform a virtual office visit. All
previous visits with weight changes are
displayed, while only visits entered through the
portal are available for editing.
When entering a virtual visit, the patient can be
requested to complete pre-defined surveys.

Diet Log The Patient Portal uses the USDA SR-16 d
atabase of foods and nutrients. The patient
selects from a list of food types and the
application will suggest common household serving
sizes. Ninety-two different nutrients can be
calculated from this information.

Patient Registration Patients can enter their inf
ormation over the Internet or an Intranet. The
registration process will guide patients through
the various screens. Upon completing the
registration screens, the information is
automatically updated on the Server Module and
available for review. The program coordinator is
notified of new registrations through an internal
system message.

Messaging Clinical staff can exchange private and
secure messages with their patients.
Messages are encrypted while in transit and
require successful authentication by the patient
in order to be viewed.
34eMD Pocket PC Module
What is eMD Pocket PC Module? The Pocket
PC Module enables the user to carry patient
information on a Windows Mobile Personal Digita
l Assistant (PDA). Information downloaded to
the PDA includes patient demographics, operation
details, complications, follow-up visits and
co-morbidities. The surgeon can enter operati
on records on the PDA and synchronize the
database with the eMD Server Module from a
remote location. The application is designed to
quickly allow entry and retrieval of data by
taking advantage of pull-down menus and check
boxes.

What is eMD eSignature Module? The eSig
nature Module provides the ability to digitally
sign electronic forms, creating a legally binding
document that can automatically be attached to
the patient chart. Using the eSignature Modul
e, entities can quickly review selected patient
documents such as HIPAA disclosures and procedure
consent forms. Digitally signed forms are sav
ed in Adobe Acrobat, a standard format for
electronic forms. An image of the signature is
displayed in the document and a digital key is
embedded in the file, providing compliance with
federal regulations for electronically signed
documents.
