    contract.
  • If you are from a CareFirst hospital there is no
    cost for enrollment.
  • If you are a hospital from Maryland or the
    District of Columbia participating as an
    Identified Participant there is no cost for
    enrollment.
  • If you do not meet either of these criteria but
    would like to participate the cost per
    collaborative team is 7,500/year.

CareFirst Delmarva Foundation 100k Lives and
Beyond Collaborative?

Submission of a letter from the hospital CEO
indicating that he/she has reviewed the
initiative requirements and agrees to the terms
of participation. Assignment of an executive
sponsor to adopt the participating ICU
team. Assembly of a multidisciplinary Improvement
Team, with a physician and improvement lead able
to commit 4 to 8 hours per week to the
project. Team commitment to collecting required
data (two culture surveys and outcomes data) and
attending the Collaborative launch and up to
three workshops per year.

Delmarva Foundation 100k Lives and Beyond
Collaborative overlap with the IHI 100k Lives
Campaign, the CMS and AHA National SCIP
Initiative, and JCAHO Patient Safety Requirements?

The Focus ICU track of the CareFirst Delmarva
Foundation 100k Lives and Beyond Collaborative is
designed to align with each of these national
initiatives. Four of the six 100k Lives
platforms are addressed (Central Line Infection,
Ventilator-Associated Pneumonia, Rapid Response
Teams and Medication Reconciliation). Two of the
four AHA and CMS SCIP focus areas are addressed
(Post-operative pneumonia prevention and DVT
prevention). JCAHO Patient Safety Requirements
related to medication reconciliation and growing
emphasis on teamwork are addressed.

  • The clinical topics for this Collaborative are
    organized into a Critical Care Bundle that has
    its origins in work shared by the Dartmouth
    Hitchcock Medical Center and the experiences of
    ICU teams from the Maryland Patient Safety Center
    (MPSC)
  • The primary clinical outcomes are prevention of
    Ventilator-Associated Pneumonia (VAP) and
    Catheter-Related Blood Stream Infections (CR-BSI)
  • We will track many evidence-based processes that
    are associated with reducing the risk of these
    infections

  • We will track many evidence-based processes that
    are associated with reducing the risk of
    hospital-acquired infections these include
  • Mechanical processes such as keeping the
    head of bed elevated and having central line
    insertion kits or carts close at hand.
  • Technique related processes such as sterile
    technique for central line insertions.
  • Treatment related processes such as providing
    treatments for DVT, peptic ulcer disease and
    hyperglycemia prevention, as well as, application
    of weaning protocols.
  • Teamwork related processes - such as
    multidisciplinary rounding with set daily goals
    and reconciling medications.

  • Culture is comprehensively addressed in all
    Delmarva Collaboratives.
  • ICUs participating in this project are expected
    to administer the Agency for Healthcare Research
    and Quality (AHRQ) Hospital Survey on Patient
    Safety Culture to all ICU staff at the start of
    the Collaborative and one year later.
  • Initiative teams will be trained to engage ICU
    staff in discussions about safety teamwork
    tools and strategies are shared and implemented
    by ICU teams.
  • Executive sponsors are expected to adopt the
    ICU team and executive rounds are conducted at
    least monthly.
  • The biggest culture intervention occurs at the
    bedside as multidisciplinary teams and regional
    workgroups take the lead on designing how they
    work so patients and healthcare workers benefit
    from a transformed healthcare infrastructure.

Foundation 100k Lives and Beyond Collaborative is

is addressed (Surgical Site Infection). Three of
the four AHA and CMS SCIP focus areas are
addressed (Post-operative heart attack ,
infection and DVT prevention).

    organized into a Care Bundles that address
    peri-operative infection, blood clot and heart
    attack risks
  • The primary clinical outcomes are prevention of
    peri-operative Surgical Site Infection (SSI),
    heart attack (MI), blood clots (DVT and PE) and
    death rates (Mortality).
  • We will track many evidence-based processes that
    are associated with reducing the risk of these
    infections.

    betadine stocks with chlorhexadine, and keeping
    patients warm.
  • Technique related processes such as avoiding
    shaving of operative sites.
  • Treatment related processes such as providing
    treatments for DVT, infection and hyperglycemia
    and heart attack prevention.
  • Teamwork related processes - such as briefing
    and debriefing procedures, coordinated flow and
    improved hand-off processes.
