6- Program Benefits
- Savings are based on income, and patients pay no
more than 25 for a typical 30-day AstraZeneca
prescription - Program offers additional savings for 60-day and
90-day prescriptions - Enrollment can be done by phone or on-line
- Prescriptions can be filled at the local retail
pharmacy - Enrollment is for the calendar year then
reapplication is required
7 Frequently Asked Questions
8 Is it possible for a patient to sign up and get
their prescriptions filled on the same
day? Yes. If the patient can answer a few
simple questions over the phone and they meet the
initial eligibility qualifications, their
enrollment will be temporarily activated, and
they may use their AZMe Prescription Savings
program for people with Medicare Part D ID number
to fill their AstraZeneca prescriptions.
9 What information will patients have to provide
in order to qualify? Their address, phone
number, social security number, income (which
must be less than 30,000 if an individual or
40,000 if a couple) and out-of-pocket expenses
which must exceed 3 of their annual house
income. Enrollees will receive a welcome kit in
the mail that includes a certification document
that they have to complete and mail in, along
with proof of out-of-pocket expenses to date. A
copy of their Part D plans explanation of
benefits (EOB) or a statement from their pharmacy
should suffice.
10 Is this a one-time fill or may they get refills
as well? Once enrolled in AZMe Prescription
Savings program for people with Medicare Part D,
patients will be able to get the AstraZeneca
medicines included in the program until the end
of the calendar year (Medicare Part D plan
year). Additionally, they will be able to get a
30-day, 60-day, or 90-day refill, if prescribed,
once the application process is complete and they
are fully eligible.
11If patients are on multiple AstraZeneca
medicines, will this program cover all of them,
or are there limitations on how many products can
be filled? This program covers all AstraZeneca
products listed on the earlier slide. For
example, if patients are on more than one
AstraZeneca medicine, they will be able to get
them for 15 or 25 each, based on their income
level, for a typical 30-day prescription.
12Will the patients have to re-enroll in the
program each year? Yes. Enrollment in AZMe
Prescription Savings program for people with
Medicare Part D expires at the end of each
calendar year (Medicare Part D plan
year). Patients who qualify again in the
following calendar year may re-enroll.
13 Does the healthcare professional need to assist
with enrollment? A healthcare professional does
not need to assist in the enrollment. Anyone can
call and speak to a personal assistant to see
about qualifying for a program. However, a
person must have a valid AstraZeneca prescription
from their healthcare professional.
14 How does a person sign up? People can simply
call 1-800-AZandMe (1-800-292-6363) and speak
with one of our assistance specialists who can
help them see if they qualify and assist with the
application process.
