• Accurate assessment

• - Encourage lying on bed/sofa for intervals
• Review Medication
• Encourage mobility/exercise

• Assess compliance with Medication
• Establish whether patient goes to bed
• Monitor fluid intake
• Ensure adequate pressure area care
• Use barrier creams/dressings to protect healthy
  skin

Skin Care

• Reduce risk of macerations
• Ensure good hygiene
• Protective skin contact dressing
• Absorbent outer dressing
• Change outer dressing frequently
• Assess for infection- treat promptly
• Seek advice from tissue viability
• Assess suitability for compression
