  ideally want to be in some time in the future.)

Plan of action

• The plan from the client should be realistic,
  desirable and appropriate in that it will bring
  about change consistent with a reduction of harm
  and or improvements in health and or social
  functioning
• Agree the plan and next steps
• Arrange next meeting
• Say goodbye
