QEH Accident and Emergency
  • Substance misuse liaison team - Team of three
  • Team leader, band 6 nurse and a specialist
    alcohol worker
  • The team operate six days per week, covering
    shift 8am until 7pm. Twilight shifts Friday and
    Saturday, 6pm until 2am.
  • Work with Drug and Alcohol 20/80 ratio
  • Project initially funded by NR to establish
    prevalence of health inequality in LBG.
  • Greenwich Teaching PCT picked up funding for 2
    years following bid by DAAT Alcohol Strategy
    Coordinator.

AE Challenges
  • AE staff not completing PAT. Only the obvious
    drinkers are picked up.
  • Training to all staff problematic due to staff
    movement.
  • Often alcohol users are in poor health once
    admitted.

A E Opportunities
  • Able to introduce detoxification regime within
    hospital
  • Improved access/care-pathways to community
    treatment
  • Good partnership working
  • Earlier identification of those drinking at
    harmful/hazardous levels
  • Data approximately 850 patients screened, ABI
    and approximately 170 drugs and 680 alcohol

GP Liaison Post
  • Initially funded via NR. Post sat in Tier 3 day
    programme/counselling service.
  • Attached to 3 GP surgerys with high prevalence
    of alcohol misuse. Delivering 11 hours of SABI
    and Extended ABIs.
  • Delivering training and information to GP
    practices and practice nurses.
  • Providing intervention tools.

Challenges GP Liaison
  • Proportion of GP practices identifying clients
    via Single Screening Question
  • On how many occasions have you used drank more
    than 6-units Women and 8-units for Men
  • Mainstreaming service so that SSQ occurs in other
    surgerys reaching a wider demographic
  • Data collection

GP Liaison Alcohol, Opportunities
  • Introduction of DES all newly registered
    patients 16 should have a short audit completed
    if positive GP to deliver 5 minute brief
    intervention.
  • Basic and advanced training available to GPs and
    practice nurses
  • Distribution of intervention tools
  • Clear care pathway (see slide
  • Data 506 slots per year offered, for SABI and
    Extended ABI, (up to 3 sessions) across three
    sites approximately 60 attendance and 70
    referral rate.

Conclusion
  • SABIs will enable clearer care pathways for
    treatment naïve service users
  • Growth of knowledge among non-specialist alcohol
    workers, e.g. Tier 1.
  • Need support from partnerships, e.g. resources,
    agreements re PAT
  • Look for what is working dont reinvent the
    wheel
