Aim

• The Western Medical Group, Inc. will redesign
  its system of care for our patients with
  diabetes. The Care Models six components will
  serve as the ultimate driving force for the
  implementation process
• Community resources policies
• Healthcare organization
• Self-management support
• Decision support
• Delivery system design
• Clinical information systems

Population of Focus

• The initial size of our population of focus will
  be approximately 125 patients. Additionally, new
  patients identified with diabetes will be added
  as they come into the practice and/or are
  initially diagnosed.
• We will focus on the patient population at the
  Benhaven Medical Center site. The system will
  first be adapted and implemented by one site in
  the corporation with a population of patients
  with diabetes.
• We will further implement this model to the
  remaining sites in the corporation. The ultimate
  goals of WMG identifies with the most recent core
  national measures of diabetes as they are
  revised.

Registry

• Registry was populated with baseline retrieval
  from Practice Management System with patient
  diagnosis of DM.
• Data Entry Process- Integrated into daily
  operations of the present delivery system and the
  PECS data entry Module (interfacing another
  application to PECS- MDServe).
• Current number of patients in registry is 110.
