CLINICAL - Assessment of needs
incorporating physical, psychological and
social aspects - Planning and implementation
of care according to identified needs -
Initiation and monitoring substitute prescribing
in conjunction with GPs - Maintenance and
Detox - Use of Motivational Interview Skills,
Cognitive Behavioural Strategies and Relapse
Prevention Strategies - Harm Reduction
Education, literature and advice - HIV/HEP
Counselling and Testing - HEP B Immunisations
- Education and support to families and carers

LIAISON Community Addiction Teams
Participation of LDS staff at CAT meetings on a
weekly basis to ensure clear lines of
communication Pharmacists Each individual nurse
liaises on a daily basis to maintain
positive communication between Pharmacist/Nursing
Staff/patient Maternity Liaison Rosie Allan
developed links with maternity staff to bridge
gaps in maternity services for pregnant drug
users. In process of setting up group with Social
Work and maternity staff to deal specifically
with supporting drug users Social Work/Substance
