Rationale for Standard 4 - Clinical Care
The clinical care provided within the NHS
environment should be of the highest quality. To
support this, robust policies, procedures and
training should be in place for all aspects of
clinical care A number of the higher risk
issues have been selected for assessment by the
NHSLA and are included within Standard 4 -
Clinical Care Through the implementation of
effective risk management policies and
procedures, the NHSLA aim to equip and empower
organisations to minimise the risks to all staff,
patients and the general public

  • The following revisions have been made to
    criteria
  • 1.4.1, 2.4.1 and 3.4.1
  • Removed
  • System to ensure the correct patient
    identification of samples, diagnostic test
    results etc.
  • Specimens etc. are incorrectly/incompletely
    labelled
  • Revisions
  • At Level 1 the focus of this criterion is all
    patients
  • At Levels 2 and 3 the focus is on inpatients only

  • The following revisions have been made to
    criteria 1.4.3,
  • 2.4.3 and 3.4.3

  • The need to document on the consent form/health
    record, details of information supplied to the
    patient (verbally and/or in other format)

  • At Levels 2 and 3 the above has been replaced
    with procedure-specific training on consent for
    staff to whom the consent process is delegated
    and who are not capable of performing the
    procedure

  • The criterion covering validation of results has
    temporarily been

    have been replaced with discharge requirements
    which are specific to each patient group

  • The focus is on the quality of the clinical
    record keeping whether hand written or electronic
  • Rewording of the requirements for clarity

    replaced with development of action plans
  • New addition

  • Blood transfusion processes

  • The focus is on the blood transfusion process

    has been spilt into two requirements
  • Process for administration of blood and blood
    products
  • The care of patient(s) receiving transfusion

  • Requirements for competency assessment of all
    staff
