administered remotely from a call center using
the telephone and the Internet (INTERxVENTOn
Call) as compared with onsite delivery
    (INTERxVENTLive)
  • Reductions in average healthcare claims in
employees who participate in the INTERxVENT
Program as compared with employees who do not
    participate

WHAT MAKES INTERxVENT LIFESTYLE
MANAGEMENT/SELF-CARE PROGRAMS UNIQUE AND
SUCCESSFUL?
14
WHAT MAKES INTERxVENT LIFESTYLE
MANAGEMENT/SELF-CARE PROGRAMS UNIQUE AND
SUCCESSFUL? (contd)
15
Rationale For INTERXVENT MD Network
  • MDs are enthusiastic about prevention. However,
MDs generally do not have the time, resources, or
infrastructure for the provision of comprehensive
    preventive services (especially lifestyle
management). MDs are generally not compensated
    for lifestyle management services.
  • INTERxVENT MD Network allows MDs to provide their
    patients with a credible, affordable,
evidence-based program without burdening them or
    their staff.

16
INTERxVENT Program Steps
  • Incorporates All Essential Components of a
    Comprehensive Chronic Disease Risk Reduction
    Program
  • Identification of At-Risk Individuals
  • Initial Evaluation (Medical History / Health
    Habits Questionnaire and Evaluation)
  • Goal Setting (Guideline-Based for BP, Lipids,
    Physical Activity, Nutrition, Weight, Blood
    Glucose, Smoking etc.)

17
INTERxVENT Program Steps
(contd)
  • Action Plan Formulation (Guideline-Based)
