District Office.

form outline or workshop handout, etc.
College coursecourse syllabus, Letter from
professor, or receipt for tuition for course.

Application Level Teachers log documenting
reading or activity, peer or administrators
observation forms, lesson plans, students
work. This period of time needs to be a minimum
of one full quarter of the school year.
Impact Level Data from multiple assessments
Teacher pre post tests, grades, textbook
tests over a period of at least one semester.
Must show an improvement in student learning.
