Context
  • The Ministry of Health and Long-Term Care is
    investing 700 million provincially over three
    years (2008 2011) to provide supports to
    seniors and their caregivers that will enable
    them to live healthy and independent lives in the
    community
  • The Toronto Central LHIN is receiving 48 million
    over three years to support local Aging at Home
    projects

Funding Allocation

Year 1 (2008/ 2009)
  • Funding was allocated to support the following
  • Additional supportive housing services in
    identified priority neighbourhoods
  • Transportation services to help seniors get to
    medical and medical appointments
  • Seniors independence programs to delay or divert
    admission to long-term care homes
  • Improving transitions for seniors from hospital
    to their homes
  • Development of a Mental Health and Addictions
    Framework to assist seniors residing in the
    community to access integrated and coordinated
    services

Year 2 (2009/ 10)
  • The Ministry of Health and Long-Term Care
    has directed that Year 2 Aging at Home funds be
    spent
  • To support resolution of Emergency Department and
    Alternative Level of Care
  • To support innovation in the community
  • To enhance access to health services for
    marginalized seniors

Objectives
  • The 2009/10 Call for Proposals builds on
    the previous years work and is focused on
    initiatives that
  • Support Ministry of Health and Long-Term Care
    provincial priorities
  • Divert seniors from avoidable long-term care home
    admission and emergency department visits
  • Continue to improve transitions for seniors from
    hospital care to care in the community.

Proposed Initiatives
  • Supportive housing for seniors with dementia or
    mental illness
  • Specialized geriatric assessments in the
    community by interdisciplinary teams
  • Crisis intervention for seniors with mental
    illness and/or addictions
  • Specialized care for seniors with dementia who
    are transitioning from acute to long-term care or
    home care.
