Purpose
  • To report the postoperative outcome and patient
    satisfaction with the new diffractive multifocal
    IOL used in eyes which underwent
    phacoemulsification

Material / Methods
  • This prospective study included patients with
    age-related cataract and unsatisfactory
    correction with glasses.
  • Information collected included uncorrected visual
    acuity (UCVA) for near and distance, best
    corrected visual acuity (BCVA), and spherical
    equivalent (SE) before and after the surgery, and
    patient satisfaction.

  • TECNIS Product Specifications (Model ZM900)
  • Modified prolate anterior surface
  • Square-edge optic
  • Diffractive multifocal posterior surface
  • UV-absorbing 6.0 mm optic
  • Polyvinylidene flouride (PVDF), CapC haptics
  • 5 D to 34 D in 0.5 D increments
  • 3-piece foldable for posterior capsular bag
    fixation

Results
  • Six hundred and forty four eyes of 363 patients
    were enrolled in the study and underwent
    implantation with the Tecnis multifocal IOL.
  • Of the 363 patients, there were 245 (68) females
    and 118 (32) males.
  • The mean age of the patients was 62 years, range
    from 47 to 91 years old (SD 8.7)
  • All postoperative visual acuity results were
    obtained at the 3-month postoperative visit.

Results
  • Preoperatively, the mean distance UCVA of these
    eyes was 0.25 (20/80) (range 0.01 to 1.00, SD
    0.36).
  • After follow-up of 3 months the mean UCVA was 0.8
    (20/25) (range 0.13 t0 1.00), significantly
    better than before surgery (plt0.001).
  • After a follow-up of 3 months a distance UCVA of
    20/25 or better was achieved by 74.2 (478 of 644
    eyes), and a distance UCVA of 20/40 or better was
    achieved by 94.5 (609 of 644 eyes).

Results
  • Preoperatively, the mean distance BCVA of these
    eyes was 0.67 (20/30) (range 0.05 to 1.0, SD
    0.29).
  • After follow-up of 3 months the mean BCVA was 1.0
    (20/20) (range 0.33 to 1.00), significantly
    better than before surgery (plt0.001).
  • The average SE at referral of these eyes was
    0.95 D (range -12.5 to 12.25 D, SD 2.28).
  • Postoperatively, after a follow-up of 3 months
    the average SE was -0.10 D (range -2.25 to 1.75,
    SD 0.48), significantly better than before
    referral (plt0.001).

Results
  • After follow-up of 3 months the mean uncorrected
    near vision acuity (UCNVA) was J1 (range to J1 to
    J4).
  • After follow-up of 3 months a near UCVA of J1 or
    better was achieved by 93.7 (604 of 644 eyes),
    and a near UCVA of J3 or better was achieved by
    99.8 (643 of 644 eyes).
  • In addition, 96.2 and 94.3 of the patients
    achieved spectacle independence for distance and
    near vision, respectively.
  • Visual phenomena was observed by 33.3 of the
    patients. Glare and halos were reported as severe
    by 3.5 of patients, moderate by 14.6, and mild
    by 15.1 of patients.

Conclusion
  • The TECNIS diffractive multifocal IOL
    demonstrated excellent near VA without
    compromising distance vision. Spectacle
    independence and patient satisfaction were high,
    whereas unwanted photic phenomena were
    clinically acceptable.
