Purpose
  • Human error is a major source of safety risk
  • YB3 made only a few references to human error /
    human factors
  • Need for practical advice to designers of
    railway systems which involve human interactions
  • Advice would be useful to other people e.g.
    independent safety assessors and safety
    regulators
  • Need for a human error application note

Scope
  • Focus on the human factors aspects of ESM
  • Guidance integrated with ESM and YB3
  • Encourage readers to consider human factors early
  • Enable readers to know what to ask, when to ask
    it, and how to get help
  • NOT a textbook on how to do human factors
  • NOT a sourcebook on tools, techniques and data

Structure content
  • For each ESM fundamental in YB3 there are human
    factors elevator statements
  • Each human factors elevator statement has a
    rationale and an application to the rail industry
  • Further reading on each area is also identified
  • Human factors activities are mapped onto the ESM
    generic lifecycle phases principal ESM
    activities
  • Checklist can be used to support assessment

Current status
  • First meeting held in April 2002
  • Ninth draft has just been issued for final
    agreement
  • Due to be published on the Yellow Book web site
    (www.yellowbook-rail.org.uk) in a few weeks
  • Good start - considered as work in progress
