2 PROJECT AND TEAM GOALS
- The goal of the project is to promote and
facilitate a cohesive approach to the education
of NHS staff and relevant organisational bodies,
across the Cheshire and Merseyside region. The
main areas of focus are -
- Diabetes
- Coronary Heart Disease
- Single Assessment Process
3Where Did The Team Come From?
- At the present time there are four education
facilitators, with more who have recently been
appointed to the team. These have come from many
different backgrounds and localities - And include
- Health visitor, Social Services, Podiatry,
Specialist Nurses in CHD/Diabetes, Pharmacist,
Public Health - Some of our team have set up and run successful
training courses for Cardiovascular disease and
Diabetes within the primary care setting
4HOW WE WORK AS A GROUP
- Fortnightly meetings between facilitators
- Monthly whole team meetings
- Communication via Blackberry, E-mail, Telephone
- Reports from meetings are disseminated through
the team and discussed/actioned
5WHAT HAVE WE DONE?
Coordinated Intelligence gathering across the
whole of the 15 PCTs
Assisted in the formation of new training and
development E.G. Community diabetes course for
district and community nurses
Met with the majority of the leads/stakeholders
in all three areas (CHD, DIABETES, SAP)
Played a key role in the facilitation of various
events
Set up a training and contact data base
Networking, Mapping and Facilitation
6THE FUTURE
- To continue to map and develop education and
training. - Maintain the training and contact data base
- Ensure that all 15 PCTs are continually assisted
in all three areas highlighted - Assist new education facilitators in their
development - Re-evaluate our own practice and project outcomes
