  • The client record form provides information
    about who starts to use support services and by
    which route they come into the service.
  • The Client Record system continues to be
    administered by the Centre for Housing Research.
    This is the same office in St Andrews who will be
    monitoring the outcome forms.

  • The SP team plan to link the information we
    receive from the client record forms and outcome
    forms to the contract information, as well as to
    the performance monitoring returns. The aim being
    to get a complete picture from the source of
    referral to the outcome for service users.

  • Providers should complete and submit client
    record forms for new clients starting to
    receive an SP service or an existing client
    starting to receive another SP service from 1st
    April 2007 using the 2007-8 Client Record Form.
  • The submitting of client record forms is a
    contractual requirement and should be submitted
    on at least a monthly basis. They should all be
    submitted electronically.

  • Client Record Forms must be completed for clients
    who start to receive the following types of
    service-
  • Supported Housing
  • Womens refuge
  • Foyer
  • Teenage parent accommodation
  • Direct access accommodation
  • Floating support service
  • Outreach service
  • Resettlement service

  • The following service types are excluded from the
    Client Record system-
  • Very sheltered housing
  • Sheltered housing with a warden
  • Almshouses
  • Peripatetic wardens
  • Leasehold schemes
  • Home Improvement Agencies
  • Community Alarm services

  • Contract managers Please ensure that your staff
    are completing the client record forms
    electronically and are using the correct service
    details as per the contract information. This
    includes the following information-
  • National Client Record Provider ID (allocated by
    CHR)
  • National Provider ID (allocated by CLG)
  • Organisation Name
  • Service Name
  • SP Service ID (e.g. S000425)
  • SP Administering Authority
